Accused ... Manny Smith
The three charges come less than a week after David Cameron hosted a Downing Street summit vowing to boot homophobia and racism out of English football.
Every club in the Premier League and Football League signed the Government's Charter to Tackle Homophobia and Transphobia.
West Ham star Ravel Morrison was last week fined £7,000 by the FA over a homophobic online remark.
The ex-Man Utd midfielder is reported to have made the comment in response to another Twitter user.
Morrison, 19, was also warned as to his future conduct.
