Everton midfielder James McCarthy recovered from the hamstring injury which kept him out of international duty with the Republic of Ireland to face West Ham at Goodison Park.
Leighton Baines was not fit enough after sustaining a similar problem with England, meaning right-sided defender Tony Hibbert came in on the left as manager Roberto Martinez's squad was stretched with three under-21s on the bench including 17-year-old Ryan Ledson.
West Ham boss Sam Allardyce made five changes with striker Andy Carroll making his first start of the season after injury, Diafra Sakho failing to recover from a back problem.
