The Brazilian centre-half is believed to have signed for £30m after passing his medical this afternoon.
On his Stamford Bridge switch, Luiz said: "I'm delighted to return to Chelsea. We had a fantastic story during my first time at the club and I want to help the team and Antonio Conte achieve a lot of success.
"I always had a wonderful relationship with the fans and I am looking forward to wearing the blue shirt at Stamford Bridge once again."
David Luiz's first Chelsea spell lasted three years between 2011 and 2014
Chelsea sold the 29-year-old to French champions PSG for £50m back in 2014.
But following a defensive hunt in the transfer window, Blues boss Conte has now officially added Luiz and Marcos Alonso to his squad.
The duo join the likes of John Terry, Gary Cahill and Kurt Zouma at the heart of Chelsea's backline.
Alonso, brought in from Fiorentina for £23m on a five-year deal, can play as a left-back but primariy features as a centre-half, meaning the Blues could have a new look pairing in central defence after the international break.
Chelsea's first game back is at Swansea on September 11, before Luiz and Alonso will make their bows at the Bridge against Liverpool five days later.
